We'll use this time to help clarify how my services can best be utilized for your personal needs, as well as to help us to decide whether or not we'd make a good fit for each other! Fee Structure and Cancellation Policy
When booking online or through me directly, your session will be prepaid in full. If the session is cancelled prior to the 24 hour window preceding the appointment, or if the practitioner needs to cancel due to an illness or emergency, the full session fee will be refunded unless the session is rescheduled. Cancellations after the 24hr window will be charged a $60 (for Full Session) or $30 (for Laser Session) cancellation fee. The remaining cost will be refunded. No-call/no-shows WILL NOT be refunded.
Confidentiality
The Life Coach/Energy Healing Practitioner recognizes that certain information of a confidential manner may be relayed during our sessions. The practitioner will not disclose this information to anyone else without specific written approval of the client, unless she is required by law to do so, such as if a) there is reason to believe that the client is in danger of hurting him/herself; b) if the client expresses an intention to hurt someone else; or c) if there is reasonable suspicion of child or elder abuse. If you have any questions or would like additional information, please feel free to ask.
Terms and Conditions
As an Energy Healing, Life Coaching, or Health Coaching Client, I consent to treatment for myself or my minor child and understand that the services provided by the practitioner, Jennifer Passavant, is intended to enhance relaxation and increase communication within my body. Energy Healing, Life Coaching, and Health Coaching are in no way to be considered or construed as psychological counseling or therapy.
I also understand that healing is a process that follows its own pace. Although Energy Healing can often accelerate the process of healing on spiritual, mental, emotional, and physical levels, it is not a “quick fix.” Everyone’s healing process is unique and specific results cannot be guaranteed.
I understand that I may experience a “healing response” during the 24 to 48 hours following the services provided.
I understand that Jennifer Passavant is not a medical doctor and does not practice medicine. She does not diagnose, treat disease or prescribe medication. I am aware that diagnosis is not given and medication is not prescribed. Instead, she assists people in correcting imbalances which in turn assists the body, mind, and spirit in unleashing their own healing abilities.
I understand that my practitioner recommends that I continue to see my regular healthcare practitioners and follow their advice.
I understand that any information exchanged during any session is educational in nature and is to be used at my own discretion.
I also understand that any information imparted during these sessions is strictly confidential in nature and will not be shared with anyone without my written permission. I do, however, give the practitioner consent to use my case history and results without using my name.
I understand that only the practitioner, Jennifer Passavant, will have access to information in my file to enhance my healing and any information received will be kept on file for 7 years.
I understand that, by NOT canceling or rescheduling my appointment prior to the 24 hour window outlined in the cancellation policy, I will still be charged for half of the session ($60 for full, $30 for Laser) while the remaining cost will be refunded, and that I WILL NOT receive a refund if I do a “no-call/no-show.”
I understand that by providing this informed consent I am assuming full responsibility for my services and I hold harmless both the practitioner, Jennifer Passavant, and the facility/location where the services are provided.
I hereby acknowledge, understand, and agree to the terms and conditions made clear here and acknowledge and understand that payment for this service constitutes a legal signature.
